• I am a staff/ retiree of TotalEnergies E&P Nigeria Limited and a member of the Cooperative.
• I agree to accept the same or any smaller number of Units in respect of which allotment may be made upon the terms of this Information Memorandum dated September 15, 2021
• I authorize you to send a Share Certificate.
• If my application is not accepted or accepted for a fewer number of shares than that for which application is made, I accept that the full application money or surplus thereof be refunded to me shortly after the approval of the allotment of the shares by the Management Committee of the Cooperative
• I accept that I have read the Information Memorandum and accept the terms of the Offer.
